手術部位感染症の予防:その現状★★

手術部位感染症(SSI)は患者ケアの質の評価指標であるという認識が、外科医、感染制御専門家、ヘルスプランナー、および一般市民の間に広まりつつある。外科医間、施設間、および国家間のSSI発生率の比較を実施すべきとの機運が高まっている。これを有意義なものとするには、データを標準化するとともに、手術を実施した施設では表在性SSIが顕在化しないことが多いため退院後サーベイランスのデータを収集する必要がある※。最適な退院後サーベイランス実施方法を明らかにするには、さらに研究が必要である。2008年に、SSIに関する2件の重要なガイドラインが米国医療疫学学会(Society for Healthcare Epidemiology of America)/米国感染症学会(Infectious Disease Society of America)および英国国立医療技術評価機構(National Institute for Health and Clinical Excellence, UK)から発表された。いずれも術前・術中・術後の患者ケアにおける重要項目について述べている。これらのガイドラインでは、手術日まで手術部位の剃毛をしないことなど、特定の時期に重要性が増す効果的な介入に加えて、血糖値、酸素分圧、体温などの生理学的パラメータが重視される傾向にある。腹腔鏡手術の普及に伴ってSSI発生率は減少しており、またメチシリン耐性黄色ブドウ球菌(Staphylococcus aureus)(MRSA)保菌者のスクリーニングおよび除菌処置は特定の外科手術に有効であるが、費用およびムピロシン耐性リスクとのバランスをとる必要がある。最終的には、SSIケアバンドルを厳格に適用することにより、理論を実践に移す必要がある。最近の研究からは、集学的アプローチによる簡便な方法がSSI発生率の減少に有効となり得ることが示唆されている。

※英国では、手術は大学病院や総合病院で実施されるが、その後の創部ケアは患者居住区の最寄りのGeneral Practitioner(GP)が引き継いで行う。よって、表在性SSIの発生および治療はGPの管轄となっており、退院後サーベイランスを実施しないと本当の発生率は不明である。
