血管カテーテルへの細菌定着の評価におけるトリプシンの増感作用

医療器具器材から分離される微生物量は、器具関連感染の可能性を評価するうえで非常に重要である。このため、本研究では、in vitroの血管カテーテルと患者から抜去した血管カテーテルを対象として、カテーテルの超音波処理前にトリプシン存在下でインキュベートすることによって表面の定着細菌量が増加するかどうかを調べた。ポリウレタン製およびシリコン製のカテーテルにin vitroで別々に黄色ブドウ球菌(Staphylococcus aureus)および大腸菌(Escherichia coli)の各臨床分離株を定着させた。次いで、細菌が定着した各カテーテルの1 cm断片を同数ずつトリプシン含有溶液またはトリプシン非含有対照溶液中でインキュベートした。さらに、カテーテル断片入りの各溶液を超音波処理後、定量培養した。臨床群でも、患者から抜去した留置カテーテルを1 cmの断片に切断し、同様にトリプシン含有溶液または対照溶液中で懸濁させ、超音波処理後、定量培養した。トリプシン処理後の超音波処理により、ポリウレタン製およびシリコン製のin vitro細菌定着カテーテルから検出される黄色ブドウ球菌はそれぞれ14倍、30倍(P = 0.03、P = 0.04)、大腸菌はそれぞれ3倍、6倍に増加した(P = 0.04、P = 0.05)。超音波処理のみと比較して、トリプシン処理後の超音波処理により、患者から抜去した留置カテーテル上に高度な細菌定着が検出される率は10%増加し、定着細菌の平均コロニー数は11%増加した(P = 0.04)。カテーテルの超音波処理前のトリプシン処理により、超音波処理後の検出感度およびカテーテルへの高度な細菌定着の評価の精度が向上する。
