Benefit and harm of iodine in wound care: a systematic review

今日では感染症と戦うことによって創傷治癒を促進する数多くの製品が利用できる。ヨードはそのような製品の1つであるが、創傷治療に対する有効性と有害作用については相反する報告がある。慢性創傷、急性創傷、熱傷、褥瘡、および皮膚移植片を対象とする27件の無作為化臨床試験のシステマティックレビューを実施した。主要な転帰項目は、創傷治癒、細菌数、および有害作用とした。その他の（殺菌）創傷ドレッシングまたは薬剤と比較して、ヨードが創傷治癒時間を短縮あるいは延長することはなかった。各試験の細菌数減少や創傷サイズの縮小について、ヨードは他の生体消毒薬（サルファジアジン銀クリーム、など）および非滅菌ドレッシングより有意に優れていたが、局所抗菌薬（Rifamycin SV MMX^R）より劣り、またヨードをアルコールと併用しても未加工の蜂蜜より劣っていると考えられた。ヨードにより甲状腺機能障害などの有害作用の頻度が増加することはなかった。現時点で得ることができる臨床試験のエビデンスから、ヨードは効果的な生体消毒薬であり、一般にいわれているような有害作用や、とくに慢性創傷および熱傷創の治癒過程の遅延は認められない。ヨードの殺菌効果は他の生体消毒薬に比べて遜色なく、創傷治癒を損なうことはない。したがってヨードは、依然として生体消毒薬としての地位を保っている。
