Twenty-four-hour observational study of hospital hand hygiene compliance

この観察研究では、2病棟における24時間にわたる医療従事者、患者、および訪問者の手指衛生遵守状況を「手指衛生の5つの機会」観察法を用いて評価した。手指衛生は医療関連感染症の減少に最も有効な対策であると考えられているが、遵守状況は最適な水準ではないことが多くの研究から報告されている。大半の研究は任意の時間帯の観察によってデータ収集を行っているが、このことは問題視されている。合計823回の手指衛生の機会（医療従事者659回、患者および訪問者164回）を観察した。医療従事者の遵守率は、医師47％、看護師75％、コメディカル78％、補助職員・その他の職員59％であった（P ＜ 0.001）。患者と訪問者の遵守率に差はなかった（56％対57％、P ＝ 0.87）。手指衛生遵守率は、医療従事者が手指衛生の5つの機会のいずれに実施したかにより異なり（P ＜ 0.001）、無菌操作前100％（3/3）、体液への曝露後93％（86/93）、患者との接触後80％（114/142）、患者との接触前68％（196/290）、患者の周囲環境との接触後50％（65/129）であった。午前中に勤務する医療従事者の遵守率が低いことが示された（P ＜ 0.001）。患者と訪問者では、手指衛生の機会と遵守率との間に関連は認められなかった。遵守率は既報の推定値と比較して高かった。医師の遵守率は最も低かったが、これは他の研究でも認められている懸念事項であることから、今後は特に介入が必要である。

繁忙期とそうでない時の比較や、重症集中病棟とそうでない病棟などの比較が加味されると、より質的な問題点が浮き彫りになるだろう。
