デバイス関連感染は集中治療室(ICU)の患者の罹患および死亡の主な原因となっている。本研究では、ICUにおけるデバイス関連感染のリスク因子を評価した。2007年7月から2008年6月に、56病院の内科ICUと外科ICU 96か所が韓国院内感染サーベイランスシステム(Korean Nosocomial Infections Surveillance System)に参加した。カテーテル関連尿路感染、中心静脈カテーテル関連血流感染、および人工呼吸器関連肺炎の発生をモニターし、デバイス関連感染率を算出した。ICUの特性と関連するデータを収集し、Poisson回帰分析を用いて統計学的解析を行った。発生率は、カテーテル関連尿路感染で1,000尿道カテーテル日あたり3.87件、中心静脈カテーテル関連血流感染で1,000中心静脈カテーテル日あたり2.23件、人工呼吸器関連肺炎で1,000人工呼吸器日あたり1.89件であった。カテーテル関連尿路感染発生率は、ソウルのICU(P = 0.032)および大規模教育病院のICU(P = 0.010)で高かった。大学病院のICUでは、カテーテル関連尿路感染発生率は低かった(P = 0.013)。中心静脈カテーテル関連血流感染発生率はソウルのICU(P = 0.001)および内科ICU(P = 0.026)で高かった。人工呼吸器関連肺炎発生率は、900床を超える病院のICUでは400~699床の病院の場合と比較して低かった(P = 0.026)。人工呼吸器関連肺炎発生率は外科ICUで高く(P < 0.0001)、また感染制御専門家1人あたりの病床数が100増加するごとに1.13倍増加した(P = 0.003)。ICUの組織や施設の特性がデバイス関連感染発生率に影響を及ぼす可能性があり、人工呼吸器関連肺炎、カテーテル関連尿路感染、および中心静脈カテーテル関連血流感染の発生率を低下させる必要がある。
