Summer influenza outbreak in a home for the elderly: application of preventive measures

ナーシングホームでは、ワクチン接種を行ってもインフルエンザのアウトブレイクが生じることがあるが、夏季の発生はまれである。2005年の酷暑の中で生じたインフルエンザアウトブレイクについて報告し、早期介入と適切な感染制御対策を進めるための迅速診断法の有用性について検討する。入居者81名（平均年齢88歳）および医療従事者48名のナーシングホームでアウトブレイクが発生し、7日間持続した。感染した入居者32例（39.5％）および医療従事者6例（12.5％）に、主要な症状として発熱、咳嗽、および喘鳴が認められた。インフルエンザが疑われ、4例の検体の迅速診断検査によりインフルエンザであることが暫定的に確認された。さらに、培養およびRT-PCR法により、入居者10名中7例のアウトブレイクが確認された。ウイルス株は、2004年から2005年の冬季の流行株であったH3N2A/New York/55/2004に類似していた。アウトブレイクが確認された時点で、地域の保健衛生当局およびナーシングホーム職員の代表者による危機管理チームが設立された。アウトブレイク制御のため、患者の隔離や、入居者・職員全員のサージカルマスク着用など一連の対策を実施した。症状を呈した入居者32例中19例および医療従事者6例中5例にオセルタミビルを治療投与し、入居者47名と残りの医療従事者42名には曝露後の予防投与を実施した。アウトブレイクは48時間以内に終息した。症状を呈した入居者の死亡率は15.6％であった。アウトブレイク前のインフルエンザワクチン接種率は、入居者で93.5％、医療従事者で41.7％であった。迅速診断検査により、速やかな対応が可能となり、感染制御対策が進められた。

迅速診断検査の活用の幅が広がり、多くの感染症で季節性の壁を越えた発生事例が報告され始めてきている。臓器別の網羅的なスクリーニング検査キットが将来的には望まれる。
