Meticillin-resistant Staphylococcus aureus screening in obstetrics: a review

イングランドでは、入院した成人を対象としたメチシリン耐性黄色ブドウ球菌（meticillin-resistant Staphylococcus aureus；MRSA）の普遍的スクリーニングが導入されつつある。現在は、通常の産科患者はスクリーニングが免除されているが、「高リスク」例および待機的帝王切開術を受ける女性はスクリーニングプログラムの対象とする必要がある。産科のMRSAに関する最新データの多くは米国からのものであるが、普遍的スクリーニングまたは対象を絞ったスクリーニングのいずれかが母や子にとって有益であるとするエビデンスはほとんど示されていない。しかし、米国では市中獲得型MRSA株が多く、MRSAの疫学は英国とは異なる。本論文では、妊娠にかかわるMRSAスクリーニングの現時点の知見をレビューするとともに、現在の診療と今後の研究への提言を行う。

監訳者コメント：
産科患者に対するMRSA保菌のスクリーニングに関する総説。この領域におけるスクリーニングの有用性を検討した研究は決して多くない。そもそも、MRSAの普遍的スクリーニングの費用対効果面からの有用性は、国によって、地域によって、患者集団によって異なるので、明確に示すのは困難である。その他の入院患者と同様に、産科患者に対するMRSAのスクリーニングの是非については今後も議論が続くことだろう。
