Patients with meticillin-resistant Staphylococcus aureus infection: twenty-first century lepers

近年は、メチシリン耐性黄色ブドウ球菌（meticillin-resistant Staphylococcus aureus；MRSA）感染症、特に市中獲得型（CA）MRSAの発生率が急速に上昇している。メディアのMRSAに関する報道の多くはセンセーショナルな取り上げ方をしており、重篤な疾患である可能性や感染力の大きさが強調されている。著者らの目的は、CA-MRSA感染症患者5例を対象とした症例集積研究により、MRSA感染に伴う精神的・社会的な被害について述べることである。MRSA感染症と診断されたことで受ける不当な扱い（stigmatization）についても分析した。患者はMRSAの診断後、自宅や職場で疎外されるなどの様々な不当な扱いを受けていることが判明した。患者は、家族、同僚、および顧客から、MRSA伝播を防止するために破格の対策を講じることを求められていると述べた。MRSAの診断を受けた結果、重要な人間関係・業務関係の崩壊・終焉なども生じていた。結論として、MRSAの診断に伴う不当な扱いにより、深刻な個人的・社会的被害がもたらされることがある。メディアおよび公衆衛生当局のMRSA感染症に関する認識には、MRSA伝播は通常は簡易な衛生対策によって防止できるという情報も必要である。

監訳者コメント：
これはWassenbergらの論文と関連するところであるが、社会の無理解も、院内で隔離を行うことの困難さを増強している。MRSA保菌に対しての説明が、患者に受け入れられがたい感情的な要因は、「よくわからないために、排除する」という人間の本質的な行動と深く関係している。お互いを知ることが理解することにつながり、差別の解消につながることは、MRSAに限らず人の宗教や文化も同様である。今後は学会や政府機関からもMRSAに関する公式情報シートを一般向けに出していくべきであろう。
