Role of hand hygiene in healthcare-associated infection prevention

医療従事者の手指は、医療環境中において、医療関連病原体を患者から患者へ伝達する最もありふれた媒体である。手指衛生は、抗菌薬耐性の拡大防止および医療関連感染症の減少のための主要な予防手段であるが、医療従事者の適切な手指衛生の遵守率は、多くの医療現場では依然として低い。本稿では、手指衛生の遵守に影響を及ぼす因子、医療関連病原体の交差感染と感染率に対する手指衛生プロモーションの影響、および手指衛生プロモーションを成功させるための重要なシステム変更である擦式アルコール製剤の一斉採用に伴う課題を取り上げる。得られたエビデンスからは、複数の介入戦略によって手指衛生の改善および医療関連感染症の減少がもたらされるという事実が浮き彫りにされている。しかしながら、特にリソースが限られた環境では、各戦略の構成要素の相対的有効性を評価し、最も有効な介入策を特定するためには、さらなる研究が必要である。世界保健機関（WHO）により発足したThe First Global Patient Safety Challengeの主な目的は、世界規模で手指衛生行動の改善を達成することであり、患者の安全を目指す強力な文化を推進することを最終目標としている。また、「医療における手指衛生に関するWHOガイドライン（WHO Guidelines on Hand Hygiene in Health Care）」に提唱された多様な戦略を実施することで発生する懸念事項と解決策についても報告する。

スイスのジュネーブ大学の感染制御の教授であるProf. Pittetは、WHOと共同で医療資源（リソース）の限られた発展途上国においても医療従事者の手指衛生を推進すべきという固い信念に基づいて、世界中でプロモーションを展開しており、まさにGlobal Infection Control Doctorである。その情熱は60歳を過ぎた現在も絶えることがなく、ますます燃え盛っている。いつか日本の感染制御系の学会に招聘したい人物である。是非一度は読んでいただきたい講演記録である。
