Do we need an ethical framework for hospital infection control?

病院における感染制御の戦略は、個人の自主性、行動の自由、または他者との接触を制限することにつながる可能性がある。医療従事者の（倫理的）実務規範は、個々の患者に対する責任を重視する傾向にある。公衆衛生に関する倫理的枠組みが対象とするのは、その大半が比較的健康で社会的能力を有する個人の集合（集団）である。病院感染制御の専門家はこの両方の視点をもち、脆弱で比較的依存度の高い入院患者集団の健康が損なわれないようにしながらも、感染者および感染性を有すると想定される患者のケアに留意する必要がある。この数年間に公衆衛生の倫理に関する一連の枠組みが提案されている。しかし、一般市民の健康の保護・促進を目指す集団的介入と、病院感染制御を背景とする介入には大きな相違があることから、病院感染制御に固有の倫理が求められる。このような枠組みを構築するためには、専門機関を設置することが最適であると考えられる。
