Emergence, transmission and phylogeny of meticillin-resistant Staphylococcus aureus sequence type 8 (USA300) in Taiwan

1995 年から 2015 年 10 月の間に台湾で採取された 5,308 株のメチシリン耐性黄色ブドウ球菌（meticillin-resistant Staphylococcus aureus；MRSA）分離株のプールから、10 例の患者の 12 株の感染分離株を含む 27 株（0.51％）の USA300 分離株が同定された。2005 年に最初の 2 つの分離株が同定され、2010 年以来 23 株の分離株が採取されてきた。系統発生解析により、地域のすべての分離株は北米の分離株に密接に関連していることと、10 例の患者由来の 13 株の地域分離株からなるクレードの存在が明らかになった。MRSA USA300 は台湾において 2005 年またはそれ以前から存在し、2010 年以来同定頻度が増加している。台湾では北米からの USA300 の輸入後に地域伝播が発生している。

監訳者コメント：
従来 MRSA は病院内で獲得する耐性菌であったが、2000 年頃から米国において黄色ブドウ球菌による壊死性筋膜炎を含む皮膚軟部組織感染症が米国内で流行し、それが PVL（パントンバレンタインロイコシジン）という毒素を産生する MRSA であることがわかり、CA-MRSA と呼ばれるようになった。一方病院で感染する MRSA は HA-MRSA と呼ばれた。この CA-MRSA の代表が USA300 と呼ばれる MRSA 株であり、SCCmec はIV 型、MLST は ST8 型であることがわかっている。北米大陸を越えて、世界中にこの株がひろがり、日本でも 2010 年頃から集団発生例がいくつか報告されている。台湾においても同様に 2005 年頃から伝播がはじまっており、USA300 株をもつ無症状保菌者が増加していることが本論論文の調査で判明している。
