インドの3病院からの院内感染メチシリン耐性黄色ブドウ球菌(Staphylococcus aureus)の皮膚・軟部組織分離株の臨床的および分子的特性

インドの3病院において、メチシリン耐性黄色ブドウ球菌(MRSA)による院内皮膚・軟部組織感染症のリスク因子を分析した。さらに、パルスフィールド・ゲル電気泳動法(PFGE)、multilocus sequence typing(MLST)、およびブドウ球菌カセット染色体(SCCmec)タイピングにより、MRSA分離株の抗菌薬耐性パターンと遺伝子型特性を判定した。黄色ブドウ球菌による院内皮膚・軟部組織感染症のために3つの3次病院に入院した患者709例のカルテを臨床的に評価した。米国臨床検査標準化協会(Clinical and Laboratory Standards Institute;CLSI)のガイドラインに従って患者の分離株の抗菌薬感受性試験を行い、マルチプレックスPCRによりメチシリン耐性とムピロシン耐性を確認した。MRSA分離株220株に対してPFGEによる分析を実施し、その結果に基づいて選択した分離株に対するMLSTおよびSCCmecタイピングを行った。MRSAが関連した感染症は、3病院でそれぞれ41%、31%、および7.5%であった。多重ロジスティック回帰分析から、MRSAによる皮膚・軟部組織感染症の独立予測因子は長期の入院[10日から19日の入院のオッズ比(OR)1.78、20日以上の入院のOR 2.83]、病院内の移動(OR 1.91)、非感染性皮膚疾患(OR 3.64)、骨髄炎(OR 2.9)、神経障害(OR 2.22)、アミノグリコシド系抗菌薬投与(OR 1.74)、およびクリンダマイシン投与(OR 4.73)であった。3病院すべてのMRSA分離株が多剤耐性であり、15のクローン(I~XV)が確認された。菌株の大半はSCCmec III型であった。最も高頻度に検出されたシークエンス型(ST)239は、PFGEによるクローンIIIにおける代表的なMLSTシークエンス型と考えられた。この主要なMRSAクローンIIIは英国由来のEMRSA-1と密接な関連があり、抗菌薬耐性が極めて強かった。インド亜大陸ではMRSAの多剤耐性クローンが拡散しているため、耐性遺伝子型を継続的に追跡する必要がある。

英国と繋がりの深い国で、英国内と同様の流行型MRSA株が存在することは興味深い。多型性解析法は沢山あるが、それぞれの重み付けは異なることから,疫学的な重み付けの判断には注意を要する。
