Survey of meticillin-resistant Staphylococcus aureus policies in UK eye departments

本研究の目的は、英国各地の眼科を対象として、日常的な白内障手術を受ける患者に対するメチシリン耐性黄色ブドウ球菌（Staphylococcus aureus）（MRSA）のスクリーニングと除菌の実施について調査することである。英国のすべての眼科を対象に郵送による質問票調査を実施し、152件中75件（49.3％）から回答を得た。63％の眼科ではMRSAに対する独自の方針を定めていた。MRSAの術前スクリーニングを実施していた眼科は50か所（66.7％）で、このうち3か所は手術を受ける患者全員に、残りの眼科は患者選択のスクリーニングを行っていた。MRSAのスクリーニングを受けた患者の割合は0％から100％で、中央値2％、平均値9.9％（95％信頼区間3.5％～16.2％）であった。全体で回答者の65.3％は、所属する科の方針は妥当であると考えていたが、一方でかなりの不満や混乱もみられ、エビデンスの欠如や日帰り白内障手術に適用可能なガイドラインの必要性に言及するコメントもあった。調査結果から、英国各地の眼科におけるMRSAの術前スクリーニングの実施にはかなりのばらつきがあることが判明した。英国保健省の現行の勧告は、日帰り手術を受ける眼科患者にはルーチンのスクリーニングは不要であると述べているが、高リスク患者にはスクリーニングを継続すべきことを示唆している。これらの勧告の科学的妥当性を確認するために、さらに調査を行う必要がある。
