手術創に対する 3 種類のデブリードマン法による細菌のエアロゾル化拡散および創部細菌汚染減少:パイロット研究

背景
創傷治療においてデブリードマンは壊死組織と創部細菌の除去に不可欠であるが、エアロゾル化による細菌伝播をもたらす可能性がある。
目的
黄色ブドウ球菌(Staphylococcus aureus)を接種した創傷モデルを用いて、以下の 3 種類のデブリードマンによる細菌伝播および創部の細菌減少について調べること。鋼鉄製キュレットによるデブリードマン、プラズマを利用するバイポーラ式ラジオ波焼灼(Coblation®)、およびハイドロデブリードマン(Versajet®)。
方法
ブタの新鮮関節標本を用いて皮膚の全層創傷を作出し、黄色ブドウ球菌を接種後、37℃、24 時間培養した。標本をキュレット、Coblation、または Versajet を用いて外科的に切除するか、未処置のままとした。各デブリードマン処置中と処置後に、エアロゾル化した細菌を能動的および受動的サンプリング法により計数した。ベースライン時、培養後、およびデブリードマン後に各創部の定量的スワブサンプル 3 つとシリンダーブラシによるサンプル 1 つを採取し、創傷の細菌量を評価した。
結果
Versajet によるデブリードマンでは著明な細菌のエアロゾル化が生じたが、キュレットおよび Coblation を用いた場合には著明な発生はみられなかった。創部の細菌量が著明に減少したのは Coblation のみであった。
結論
感染・保菌創に Versajet によるデブリードマンを行う場合は追加的防御策を実施するべきである。キュレットや Coblation を用いる場合は、同様の予防策の必要性は小さいと考えられる。
