Sero-epidemiology of hepatitis A virus infection among healthcare workers in Korean hospitals

韓国ではA型肝炎ウイルス（HAV）の報告が増加しており、韓国の医療従事者におけるA型肝炎ウイルス（HAV）のアウトブレイクも増加している。2008年に実施した本研究では、韓国の4病院の医療従事者3,696名のHAV感染について血清疫学の評価を行った。医療従事者を対象として、市販のキットを用いて抗HAV IgG抗体を調べた。人口統計学的特性、職業、勤務地、他の肝炎ウイルスの血清学的状態などのデータを収集した。統計学的解析を行い、HAV血清陽性に関連する因子を特定した。対象者3,696名のうち、女性は2,742名（74％）で、大部分（96％）が20～39歳であった（中央値28歳、範囲19～68歳）。職種の内訳は医師18％、看護師46％、看護助手10％、パラメディカル技術者11％、事務職員15％であった。HAV血清陽性率は年齢とともに有意に上昇し（P < 0.001）、24歳以下1.8％、25～29歳14.7％、30～34歳41.8％、35～39歳75.5％、40歳以上93.7％であった。20～39歳の対象者では、年齢区分別のHAV血清陽性率は医師が他の職種群と比較して有意に低かった（P < 0.001）。韓国では、29歳以下の医療従事者に対する集団ワクチン接種、または30～39歳の医療従事者に対する血清スクリーニングと非免疫者へのワクチン接種を、特に医師を対象に考慮する必要がある。
