Post-discharge surveillance to identify colorectal surgical site infection rates and related costs

入院患者のデータのみに基づくことの問題点に注目したサーベイランス研究が増加しつつある。退院後サーベイランスデータが欠けていると、手術部位感染症（SSI）発生率と負荷を過小評価することになる。英国では、大腸手術の退院後サーベイランスデータは発表されていない。この領域のSSI発生率は極めて高いと考えられ、最も治療コストを要する領域の1つであるため、研究対象として重要である。今回、電話面接と家庭訪問による30日間のサーベイランスプログラムを用いて、大腸のSSIに関する研究を実施した。SSI患者が使用した医療資源の増加分を記録し、コストを算出した。組み入れ基準に合致し、30日間の経過観察を終了した105例中29例（27％）がSSIを発症し、このうち12例は退院後に診断された。SSI発症までの平均日数は13日であった。多変量ロジスティック解析から、体格指数（BMI）が唯一の有意なリスク因子であった。感染患者1例の治療に要した増加コストは10,523ポンドであったが、このうちの15％はプライマリ・ケアから支払われた。5か月間のサーベイランスプログラムの実施には5,200ポンドを要した。サーベイランス看護師の作業負荷の分析から、看護師は看護補助者（healthcare assistant）で代替可能であることが判明した。大腸手術後のSSI発生を検出するための退院後サーベイランスは、正確な増加コストの完全なデータを得るために必要である。

監訳者コメント：
皮肉にも、わが国は術後の平均在院日数が長いため、専門家の間では日本のSSIサーベイランスのほうが精度が高いと考えられている。いずれにせよ、術後30日までフォローするのは大変なことである。
