背景
長期ケア施設における医療関連感染症と抗菌薬使用に対する認識が高まりつつある。2017 年、イタリアにおいて第 3 回全国点有病率調査が、第 3 回 Healthcare-Associated Infections in European Long-Term Care Facilities(HALT3)研究の一環として実施された。
目的
HALT3 研究の結果を報告し、長期ケア施設の入居者集団、実施された優れた行動、感染症の有病率および抗菌薬使用の実施率について分析を行うこと。
方法
本調査は点有病率調査としてデザインされ、2017 年 4 月から 6 月に実施された。実施施設に居住入居している入居者全員を対象とした。すべての施設に対して、施設に関する質問票、全入居者の入居リスト、ならびに活動性感染症の徴候/症状を有する入居者および/または抗菌薬投与を受けている入居者に関する質問票に記入して回答するよう求めた。
結果
全体で、418 施設が本研究に参加した。入居者 24,132 例が適格とされ、ほとんどが 85 歳超で、失見当識および失禁を有していた。医療関連感染症の有病率は 3.9%で、施設の 50%は感染制御の専門トレーニングを受けたスタッフを有していると報告した。感染症の 26.4%のみが微生物学検査用サンプルによって確認されており、分離された微生物の 26.9%は少なくとも 1 クラスの抗菌薬に対する耐性を示した。全体で、入居者1,022 例が少なくとも 1 種類の抗菌薬を投与されており、セファロスポリン系薬が最も多く処方されていた。
結論
本研究において、実施されていた感染制御および抗菌薬管理の方策の数はこれまでの研究と比べてかなり多かったことが示された。このことは、医療関連感染症有病率、抗菌薬使用、および抗菌薬耐性の低減につながる可能性がある。これらの側面についてモニタリングを行うために、さらなる研究が必要である。

監訳者コメント
長期療養施設における医療関連感染症はその現状すらよく分かっていないことから、非常に懸念されている。しかし本研究では参加した施設の半数に感染制御の専門家を有し、医療関連感染症の有病率も3.9%と一般的な病院における有病率よりも低かった。しかしこれらはあくまでもイタリアの研究であり、日本でどうなのかは調べてみないと分からない。点有病率調査は比較的労力の少ない調査法であり、日本でも試してみる価値はあるのではないだろうか。
