結核は医療従事者の職業関連疾患である。先進国の病院では、管理的介入と工学的介入を同時に実施することにより結核院内感染リスクが低下している。著者らは、感染制御管理対策が、資源の限られた流行国の医療従事者における潜在性結核感染症のリスクに及ぼす影響を調査した。医療従事者全員に連続的ツベルクリン反応検査(TST)がルーチンに提供されているリオデジャネイロ市内の大学附属病院で、介入研究を実施した。1998年10月から2001年2月にかけて、結核疑い例および結核確定例の入院患者の隔離、抗酸菌喀痰検査の迅速な結果報告、および防御用レスピレータ(監訳者註:N95マスク)の使い方に関する医療従事者の教育といった感染制御対策を順次実施した。初回検査を実施した医療従事者1,336名の中で599名に再検査を行った。これらの対策の実施中および実施後の1,000人・月あたりのTST陽転率は、5.8/1,000人・月から3.7/1,000人・月へ減少した(P=0.006)。高い減少率が認められたのは集中治療部門(20.2から4.5、P<0.001)、臨床病棟(10.3から6.0、P<0.001)であった。陽転率の減少は医師と看護師で最も大きかった(それぞれ7.6から0、P<0.001;9.9から5.8、P=0.001)。結論として、流行国においても感染制御管理対策によって医療従事者の潜在性結核感染症が有意に減少する可能性があり、工学的感染制御対策のための資源が限られている場合であっても、管理対策を実行すべきである。

監訳者コメント:
金がなければ人手を使え、ということか。目新しいアプローチではないが、まじめにやることが重要であると示唆する論文である。なお、ツベルクリン反応陽転については潜在性結核感染症としてとらえるのが世の流れである。かつては化学予防と呼ばれた陽転者への抗結核薬の投与も、現在は潜在性結核感染症の治療と位置づけられている。
