Risk factors for infection in patients undergoing shoulder arthroscopy: a systematic review and meta-analysis

肩関節鏡視下手術後の感染症リスクの究明および感染症のリスク因子の特定を目的としてメタアナリシスを実施した。PubMed/Medline、Embase、Cochrane Library のデータベース、ならびに既報のシステマティックレビューおよびメタアナリシスの引用文献リストを系統的に検索し、さらにマニュアル検索を実施した。ランダム効果モデルを用いて、サンプルサイズ、Egger 検定の P 値、研究間の不均一性に基づき統合オッズ比（OR）を推定した。 選択した論文 29,342 報のうち、16 報（患者 74,759 例）の後向き研究を対象とした。質の高いエビデンスでは、糖尿病（OR 1.30、95％信頼区間［CI］1.20 ～ 1.41）、高血圧症（OR 1.26、 95％CI 1.10 ～ 1.44）の患者で感染症リスクがより高いことが示され、中等度の質のエビデンスでは、肥満（体格指数 ≥ 30 kg/m²）（OR 1.42、95％CI 1.28 ～ 1.57）、男性であること（OR 1.65、95％CI 1.12 ～ 2.44）、American Society of Anesthesiologists（ASA）クラス ≥ 3（OR 2.02、95％CI 1.02 ～ 3.99）、喫煙歴（OR 2.44、95％CI 1.39 ～ 4.28）の患者において、感染症リスクがより高かった。メタアナリシスでは、年齢、手術時間、または飲酒量と、感染症との関連は認められなかった。このメタアナリシスにより、肩関節鏡視下手術後の有意な感染症リスクとして、糖尿病、肥満、高血圧症、男性であること、ASA クラス、喫煙歴の 6 因子が特定された。これらの患者関連リスク因子は、肩関節鏡視下手術後の感染症リスクがより高い術後患者を特定するのに有用と考えられる。
