Management of infection control in dental practice

本研究は、179件の歯科診療所の感染制御および器具消毒に関連するマネジメント方針および手順について、訓練を受けた調査チームが実施した観察研究である。種々の感染制御対策のマネジメントに関する情報、特に歯科器具の消毒に関する情報を、面接および行動指針文書の調査により収集した。本研究から、大半の診療所（70％）には感染制御に関するマネジメント方針があると述べていたが、文書化されていたのはこのうち50％のみであることが示された。感染制御方針については、79％がBritish Dental Association Advice Sheet A12を利用していた。89％の診療所が感染制御手順があると述べていたが、文書化されていたのはこのうち62％であった。職員の77％が特定の感染制御訓練を受けたと述べていたが、器具消毒に関する訓練は主にデモンストレーション（97％）または実技観察（88％）の形式で実施されていた。多くの歯科看護師（74％）および歯科医（57％）は、単回使用器材であることを示す記号を知らなかった。感染制御または汚染除去の実践の監査を実施していた診療所は11％であった。大半の診療所には、歯科診療所による感染制御マネジメントのための方針および手順があったが、多くは文書化されていなかった。感染制御およびその文書化に関する職員訓練のマネジメントは不十分であり、歯科診療所用の品質管理システムの開発を検討する必要がある。

本研究は英国の実情を紹介したものであるが、日本をはじめ各国とも歯科診療所が小規模であるがために感染制御の指針作成や職員訓練などに問題を抱えていると思われる。示唆に富む論文である。
