Antibiotic use in two cohorts of German intensive care units

ドイツの集中治療室（ICU）2集団における抗菌薬の使用を評価した。第1の集団は、抗菌薬使用および耐性菌のデータを四半期ごとに収集し、フィードバックを行うサーベイランスプログラムに参加しているICU（34カ所）。第2のICU集団は横断的研究として、ドイツ南西部の病院（58カ所）をサンプルとした。抗菌薬投与量については2種類の用量の定義を使用した。それは、世界保健機関（WHO）のATC分類（Anatomical Therapeutic Chemical Classification）2001年版の1日規定用量（DDD）の定義、および入院患者に処方されている非経口薬剤の用量をより正確に反映する1日推奨量（RDD）の定義である。データはのべ100患者・日あたりの1日規定量あるいは1日推奨量で示した。病院の規模、病院の所属、研究実施年、およびICUの種類が、抗菌薬全使用量に影響するかどうかを判定した。いずれの用量定義を用いても、2つのICU集団間で抗菌薬全使用量に差がみられた。抗菌薬の使用量が多いことは、主として病院の所属（大学病院対非大学病院）および病院の規模に関連していた。非大学病院のICUにおける抗菌薬全使用量の平均値は、病院の規模別カテゴリーによりのべ100患者・日あたりDDD 106～111（のべ100患者・日あたりRDD 59～67）の範囲であったのに対し、大学病院のICUではDDD 140（RDD 87）であった。結論としては、ICUのコホート間で抗菌薬の使用量を比較し経時的傾向を評価するためには、大学病院であるか否か、および病院の規模でデータを補正する必要がある。

抗菌薬使用量を評価する興味深い試みである。大学病院で抗菌薬使用量が多い傾向が認められた点も注目される。
