Evidence of aerosolised floating blood mist during oral surgery

歯科手術には歯科用タービン、エアーモーター、マイクロエンジンハンドピースなどの高速器具を使用し、大量の飛散物や浮遊粒子が発生するが、それらは口腔内の微生物で汚染されている場合がある。このような微粒子ミストは血液成分を含んでいると推測される。本研究では、高速器具を使用した口腔外科手術を実施する部屋で、血液で汚染されたエアロゾルが存在するかどうかを調べた。口腔外吸引装置でサンプルを採取した（132例）。実験にあたっては、吸引装置のノズルに不織タオルをフィルターとして設置し、目にみえないミストを手術の場所から20、60、100 cmの距離で採取した。それぞれの抜歯後にフィルターに対してロイコマラカイトグリーンによる推定試験を行った。血液推定試験で陽性の粒子は、手術の場所から20 cmの位置で76％、100 cmの位置で57％であった。この結果から著者らは、血液汚染物質が血液汚染エアロゾルとなって空中を浮遊する可能性があると考える。以上の結果は、歯科診療時には免疫不全患者だけでなく健常者にも、交差感染のリスクがあることを示唆している。

歯科処置には高速で回転する器具を頻繁に使用し、周囲への飛沫・エアロゾルの発生が懸念される。そのことを定量的に示した論文である。
