病院敷地内のビル解体中の粒子状物質および真菌胞子の環境サンプリング調査★

病院に隣接した解体作業や改築作業には真菌の空気感染のリスクがある。2005年11月から2006年3月に、エッセン大学病院の敷地内の3階建ての古い建物が解体された。塵埃曝露を予防するために、不透水性プラスチックホイルで建物を覆い、掘削機を用いて建造物を機械的に破砕した。水噴射により塵埃放出を最小限に抑えた。解体作業に伴う塵埃放出により患者に感染リスクが生じるかどうかを判定するために、解体前と解体中の空気中の粒子濃度と真菌濃度を測定した。建物周囲の7か所で空気サンプリングを行い、同時に気象状況を監視した。超微粒子、および0.3 μm以上、0.5 μm以上、1 μm以上の粒子の濃度は、解体中のほうが解体前よりも有意に高かったが、その倍率は低かった(超微粒子1.6倍、0.3 μm以上の粒子1.6倍、0.5 μm以上の粒子2.9倍、1 μm以上の粒子3.3倍)。37℃で培養可能なかび類の濃度は両期間で差はなかった(解体前の中央値66 cfu/m3、解体中の中央値80 cfu/m3)。22℃で増殖したかび類の濃度は、気温および湿度と有意な相関を示し、解体前(中央値510 cfu/m3)のほうが解体中(中央値210 cfu/m3)より有意に高かった。結論として、病院敷地内での解体作業中の真菌による患者の感染リスクは、防御策が実施されていれば上昇しない。

監訳者コメント:
発生源での塵埃対策をするというまさに逆転の発想である。医療施設内での防護と合わせると最も効果的な予防策といえよう。
