医療に使用可能な現行の殺芽胞薬とその限界★

これまでクロストリジウム・ディフィシル(Clostridium difficile)は、環境汚染および環境を介する二次伝播をもたらす可能性のある病原体として認識されてきた。残念なことに、医療環境で現在用いられている多くの消毒薬は C. difficile に対して無効である。たとえば、アルコール含有擦式手指消毒薬には殺芽胞効果がなく、一部の四級アンモニウム化合物や洗浄薬は芽胞形成を促進することさえある。消毒薬の効果の検査法の標準化を図るために欧州標準が作成されているが、医療環境における C. difficile に対する消毒薬の効果の検査法については欧州標準が存在しない。多くの検査施設では、食品、家庭、および製造業に適用するために策定された EN 13704 修正版を使用している。消毒薬の検査の際に重要なのは、清潔状態だけでなく汚染状態でも行うこと、および消毒薬が適切に中和できることである。現行の殺生物薬は C. difficile に優れた効果を有するものもあるが、比較的長時間の接触を必要とする場合があり、この点が実際の使用時に正確に反映されていないことがある。薬剤の中には 1 時間接触させても C. difficile に対する効果をほとんど示さないものもある。C. difficile に対する効果の試験は技術的に非常に困難であること、またこのために検査結果の施設間差が大きい可能性があることを認識しておく必要がある。データを解釈する際には、使用した検査方法を詳細に検討することが不可欠である。

抗菌療法関連下痢症の起因菌として臨床的に最も重要なクロストリジウム・ディフィシル(C. difficile)は、偏性嫌気性菌でありながら、芽胞を産生することから環境を介して水平伝播することとなり、病院感染制御の観点からは最も対応が難しい病原体の 1 つである。医療環境における環境表面の汚染にどのように対処するのか、現場における重要な課題となっている。
