Quality of alcohol-based hand disinfectants and their regulatory status: Development and marketing authorisation

2005年の調査で、欧州連合（EU）には手指消毒薬に少なくとも4種類の法律上の製品分類、すなわち医薬品、殺生物製剤、化粧品、医療器材があることが示されている。国際的な統一分類はなく、これらの製品の法的位置づけは国ごとに定められている。法令遵守を推進するために、4種類の製品分類ごとに異なるレベルの公的サーベイランス方法が定められているが、これには個々の製品ごとの販売承認・製造施設の監査から、特定の一般ガイドラインに準拠した作業の義務に至るまで、様々なものがある。
殺生物製剤に関する法令は、環境毒性学および毒性学の領域を対象としているが、利用者側の品質や有効性に対する要望については、それほど指向していない。一方、医薬品に関する法令は最も大規模なものであり、品質、安全性、有効性、および関連当局による独立したリスク便益評価を義務づけている。規模に関しては、残り2種類の製品カテゴリーである化粧品と医療器材は、殺生物製材と医薬品の中間に位置する。医療器材では、製品の品質、安全性、および性能が規定の要件に適合しているかどうかを確認し、第三者機関の管理のもとで適切な品質保証システムを運用するのは、製造者の責任である。化粧品では、ある程度の法的規制はあるものの、その枠の中で、製品の安全性および要件への適合を確認することのみが、製造者の責任である。
本論文では、この複雑性を増しつつある状況から脱する方法、すなわち、特定の販売国とは無関係に、利用者の要望と法的要件をともに満たすような単一の品質規格の策定について述べる。製品に関するこの国際的な品質規格は、利用者の利益という点で、各国の個別の基準を上回るものと思われる。
