Modification in prescribing practices for third-generation cephalosporins and ciprofloxacin is associated with a reduction in meticillin-resistant Staphylococcus aureus bacteraemia rate

メチシリン耐性黄色ブドウ球菌（MRSA）による院内感染症発生率を低下させるために、シプロフロキサシンおよび第三世代セファロスポリン系抗菌薬の静脈内投与を自制する教育的介入研究を実施した。2か月間の介入プログラムの実施前18か月間と実施後16か月間の病院および集中治療室それぞれのMRSA菌血症発生率を観察し、その効果を評価した。病院全体のMRSA菌血症発生率は、本研究の終了時点で62.9％低下し（P＜0.001）、MRSA保菌率は38.4％低下した（有意差なし）。本研究期間中、メチシリン感受性黄色ブドウ球菌（MSSA）菌血症エピソードが同時に減少することはなかった。シプロフロキサシンおよび第三世代セファロスポリン系の院内調剤用量は、それぞれ80.4％および75.2％の減少を示した。集中治療室のMRSA血流感染症の全発生率は低下したが（1,000床・日当たり4.200対0.272）、有意差は認められなかった。

監訳者コメント：
抗菌薬の処方がMRSAのprevalenceに影響を与える研究は多数出されているが、その中から統計的に因果関係が強いとされる第三世代セファロスポリン（英国ではケファロスポリンと発音する）とシプロフロキサシンについて、処方量とMRSA分離率の相関を検討した研究である。あまりにもシンプルな比較すぎて、そのほかの交絡因子（confounders）がないかどうか、かんぐってしまう論文である。Study designから見れば、エビデンスレベルはやや弱いように思う。
