急性外傷性創傷におけるポリヘキサメチレンビグアナイド(PHMB、ポリヘキサニド)0.04%の有効性を評価することであった。61 例の患者を対象に、無作為化二重盲検プラセボ対照前向き試験を実施した。ポリヘキサニドを用いて処置を行った群は、処置後 60 分でベースラインと比較して有意に菌数(log10 コロニー形成単位)を減少させたのに対し(P < 0.001)、リンゲル液群は 60 分間に有意な変化を示すことはなかった。急性外傷性創傷において、ポリヘキサニド 0.04%を用いた処置は細菌数の有意な減少をもたらした。

監訳者コメント
消毒薬であるポリヘキサニドは、国内ではコンタクトレンズの洗浄剤に含有されていることで有名である。創傷にも安全に使用可能とされてきたが、創傷感染の中でもどのような状況で有用であるかははっきりしていなかった。本検討では救急外来を受診したものの、全身症状を伴うほどではない患者を対象にランダム化比較試験を行ったが、結果的に汚染が少ない患者が多くを占めていた。今回とは創の状態、場所、汚染状況が異なった集団において、本剤の効果を追試した結果が待たれる。
