Epidemiological investigation after hospitalising a case with pandrug-resistant Acinetobacter baumannii infection

2000年代前半に台湾で汎薬剤耐性アシネトバクター・バウマニー（Acinetobacter baumannii）（PDRAB）が出現したが、小児病院（病院A）でこの菌が初めて同定されたのは、患者1例が呼吸器病院（病院B）から移送された2005年3月であった。PDRABは入院時に採取した眼のスワブ標本から回収された。培養結果の判明を受けて、両病院で感染制御予防策に加え疫学調査を実施した。入院患者30例（病院Aの7例と病院Bの23例）、医用機器、および病棟環境から合計212件の標本を採取した。A. baumannii陽性は、病院Aの標本84件中13件（15.5％）、および病院Bの標本128件中23件（18％）であった。このうち、病院Aの患者2例と医用機器から採取した分離株6株、および病院Bの患者3例から採取した5株がPDRABであった。患者1例は両病院に入院しており、それぞれの病院で1株のPDRAB分離株が検出された。A. baumanniiの36分離株には、9のIRS-PCR（infrequent restriction site-polymerase chain reaction）型および12のパルスフィールド・ゲル電気泳動（PFGE）型が認められ、6つの抗菌薬耐性型が同定された。1つの主要なIRS-PCR型（4つのPFGE型）に25分離株が属し、汎薬剤耐性（PDRABの11分離株はすべてこの型）または多剤耐性（イミペネムのみに感受性を示す）のいずれかを示した。A. baumanniiは、患者およびその周辺機器から分離される、普遍的な微生物である。両病院にはA. baumanniiの多剤耐性または汎薬剤耐性のクローンが蔓延していた。

海外では多剤耐性のアシネトバクター・バウマニーが多く検出されており、近隣諸国でも台湾や韓国でその分離頻度が増している。わが国でも今後監視を強化していく必要がある。
