Extended-spectrum β-lactamase (ESBL)-producing enterobacteria: factors associated with infection in the community setting, Auckland, New Zealand

オークランド市中での基質特異性拡張型β-ラクタマーゼ（ESBL）産生腸内細菌の疫学を記述し、症例対照研究デザインを用いて感染に関連する因子を特定することを目的とした。感染患者107例からESBL産生腸内細菌を分離し、そのうち98例（92％）から人口統計学的データおよび臨床データが入手できた。優勢な菌種は大腸菌（82％）であり、その分離源で最も多かったのは尿（97％）であった。単変量解析より、ESBL陰性腸内細菌に感染した対照群と比較して感染に有意に関連していた因子は、居住型介護施設への入居、M病院への最近の入院、最近の抗菌薬使用、高齢（75歳以上）、導尿カテーテル留置、および慢性閉塞性肺疾患（COPD）、心血管疾患、神経疾患、および再発性尿路感染症の併存歴であった。多変量解析でも、居住型介護施設への入居とCOPDは有意な関連因子であった。また、パルスフィールド・ゲル電気泳動法によるESBL産生大腸菌の型別により、1種類の共通株を特定した。居住型介護施設入居とCOPD歴が、オークランド市中でのESBL産生腸内細菌感染と有意に関連していると結論される。居住型介護施設に複数の空間クラスターがみられること、および共通株の存在から、点発生源によるアウトブレイクが示唆された。市中患者の相当数が、居住型介護施設に入居しておらず、最近入院してもいなかったことから、さらに広域でのESBL産生腸内細菌の独立した発生が示唆される。
