Laboratory diagnosis of Clostridium difficile-associated disease in the Republic of Ireland: a survey of Irish microbiology laboratories

2006年、Health Protection Surveillance Centre（HPSC）はクロストリジウム・ディフィシル（Clostridium difficile）に関するアイルランドの国内ガイドラインを作成するための作業グループを設立した。C. difficile診断業務の現状を明らかにするために、臨床検査室に質問票を送付した。34の病院のC. difficile診断業務を請け負っている44の検査室中、29カ所から回答を得た。29の検査室中、25カ所（86％）でC. difficile検査のための試料の処理を行っており、4カ所（13.8％）は試料を他の検査室に転送していた。16の検査室（64％）は他の医療施設の試料の処理を行っていた。便のC. difficile検査をルーチンに行っていた検査室はなく、依頼された場合のみ試料を検査する検査室が7カ所（28％）、また、全水様便検査時（39％）、全院内感染下痢症（44％）、特定の臨床基準（28％）、抗菌薬治療歴あり（22％）などの特定の選択基準を採用していた検査室が18カ所（72％）であった。すべての検査室が種々の酵素免疫測定法を用いて便のC. difficile毒素を直接検査していたが、このうち24カ所（96％）は毒素AとBの両方、1カ所は毒素Aのみの検出を行っていた。3カ所（12％）は細胞毒性検査を実施していたが、PCR法を使用していた検査室はなかった。6カ所（24％）はC. difficileの培養を行っていたが、特定の場合に限られていた。7カ所（28％）はアウトブレイク時に分離株のタイピングを行っていたが、自施設に検査設備を有する検査室はなかった。HPSC作業グループは、C. difficile感染症を対象とした検査診断、サーベイランス、および管理の国内勧告を作成する予定である。国内の診断業務には顕著な相違があり、また国の標準検査施設^※がないため、これらの勧告の実施にあたっては対処すべき費用上の問題があると考えられる。
