Mycobacteria and allograft heart valve banking: an international survey

1970年代以降、多くの組織バンクが同種心臓弁でヒト型結核菌の検査を行っている。1980年代に結核リスクが低いドナーの選択法が導入され、結核感染リスクが減少しているようである。規制指針（regulatory guidance）には結核検査に関する規定はないが、最近の結核歴を有するドナーは除外している。国際的な心臓弁バンクが実施した調査により、ドナー選択および心臓弁の検査と処理にばらつきがあることが明らかになった。調査対象のバンク（欧州および米国）の報告によると、15年間でドナー38,413例から心臓弁組織が提供され、そのうちドナー32,289例に結核検査を実施したが、陽性例はなかった。ドナー27,840例の心臓弁組織を染色し、顕微鏡検査を実施したが、抗酸性桿菌陽性例はなかった。非結核性抗酸菌は心臓弁24個の培養で検出された。非結核性抗酸菌のクラスターが検出された場合は、是正措置および予防措置を促進するために、心臓弁バンクがドナー選択に際して結核リスクを有するドナーの除外、検体の抗酸菌培養、および発生源の調査を行うことを推奨する。

臓器移植と感染症というと、硬膜製品のライオデュラがクロイツフェルト・ヤコブ病で有名である。心臓弁と抗酸菌の関係はあまり知られていない。本論文により、非結核性抗酸菌が一定の割合で検出されていることがわかり、驚きである。
