A standardized protocol for perioperative antibiotic prophylaxis is associated with improvement of timing and reduction of costs

本研究の目的は、周術期抗生物質予防投与（PAP）の単一のガイドラインを実施し、PAPの投与時期と費用への影響を評価することである。ランダムに選択した処置について薬剤の選択と投与時期を、PAPの適用に関する新規ガイドラインの実施前後で比較することによって、ガイドライン実施の効果を評価した。介入前に、異なる診療科の処理153件を観察したところ、8種類の抗菌薬を様々な用量で使用しており、これらの処置の20％では切開後にPAPを投与していた。介入の2カ月後に147件の処置を観察したところ、投与した抗菌薬は3種類で、いずれも正しい用量で使用されていた。切開後のPAP投与は20％から7％に有意に減少した（P＝0.002）。改定PAPプロトコールにより、質の改善に加え、1年間で正味112,000 USドル以上の費用節減がもたらされた。本研究は、単一で簡明なPAPプロトコールを実施することにより、用量と投与時期が改善することを示している。PAPの費用も削減された。

監訳者コメント：
ランダムに投与されていた予防投与薬を、プロトコールを作成したら、用法用量が改善され、コストも削減になった、というシンプルな論文。これで感染率がどうかまで加えてあると、★がついたのですが・・・。
