伝染性海綿状脳症は、一般にプリオン病として知られる一群の致死的な神経変性疾患である。プリオン病は従来の不活化法に抵抗性があり、複数の人体への使用を通して外科用機器によって医原性感染を起こす可能性がある。このような性質のためにプリオン感染因子の適切な検出法が必要であり、外科用機器の安全性を確実なものにするプリオンの消毒法の開発への要求が高まってきている。感染性の評価法としては、他の技術も利用されているが、動物を用いたバイオアッセイ(生物学的検証)が依然として「gold standard」である。外科用機器の大半がステンレス製であるため、その表面を動物を用いたバイオアッセイで検査する場合は、極細の外科用ステンレス製ワイヤーが広く用いられている。これらのワイヤーは埋込みが容易であり、また動物への再移植の際に残留物質の溶出が不要である。しかし、外科用機器とは容積、形状、大きさが異なるため、比較対象としての妥当性は疑問である。本稿では、このような汚染されたワイヤーの洗浄(残留蛋白質6.3~16.0 ng/mm2)が、平らな金属表面の洗浄(残留蛋白質63.9~89.3 ng/mm2)と比べてどの程度容易であるかを、推奨される洗浄剤を用いた比較により示した。外科用機器由来のプリオン感染因子の除去あるいは不活化を検討する実際的な手段として、ワイヤーを利用するには注意が必要であることを、これらの結果は示している。

監訳者コメント:
プリオンの不活性化については、未だ決定的な推奨は出てきていない。本邦にある他の学会からプリオンの不活性化についての緊急勧告が出されているが、これさえ行っておけば大丈夫という誤解が発生しないよう、知識の正しい普及のために、感染制御の専門家の働きかけがますます重要になってくるであろう。
