Cost of the meticillin-resistant Staphylococcus aureus search and destroy policy in a Dutch university hospital

2000年から2004年のマーストリヒト大学病院の黄色ブドウ球菌菌血症に対するsearch and destroy戦略および治療に関連した費用を算出した。Search and destroy戦略の費用対効果の損益分岐点をモデル化により判定した。1年あたり平均22,412例の患者が平均8.7日間入院していた。メチシリン耐性黄色ブドウ球菌（MRSA）のスクリーニングを1年あたり246例に実施し、そのうち74例が除菌の対象となり、予防隔離下で治療した。血液培養陽性から算出した当病院のMRSA有病率は0.7％であり、黄色ブドウ球菌血流感染症患者全員の平均入院期間は39.9日であった。当病院でMRSAに対する前向き調査に要した費用は1年あたり1,383,200ユーロ、MRSA予防と黄色ブドウ球菌血流感染症の治療に要した費用は2,736,762ユーロであった。MRSAとメチシリン感性黄色ブドウ球菌（MSSA）の種々の比率をシミュレーションしたところ、MRSAの有病率が8％に達しても、予防費用は黄色ブドウ球菌感染症の治療費用より少ないことが判明した。結論として、当病院のsearch and destroy戦略の総費用は、黄色ブドウ球菌血流感染症の治療費用より少ない。MRSAの有病率が8％以下であれば、search and destroy戦略は費用対効果に優れる。経済的観点から、search and destroy戦略は院内のMRSA有病率を1％未満に維持するための最善の選択肢である。

有病率が8％までは、Search & Destroyが有効という試算を表した論文である。それではっきりしたことは、これをそのまま輸入しても採算が取れないということである。しかし諦めなくとも、部分的に能動的サーベイランス培養（ASC）を取り入れていくための参考にはなる。
