目的
カルバペネマーゼ産生腸内細菌科細菌(carbapenemase-producing Enterobacteriaceae;CPE)のハイリスク診療科に基づくスクリーニングから生じる隔離の需要および検出された CPE 負荷の潜在的割合を推定すること。
方法
ロンドンの 3 病院の臨床診療科グループを大腸菌(Escherichia coli)およびクレブシエラ(Klebsiella)属菌のカルバペネム耐性の発生率により順位づけた。接触予防病床利用日を以下の 3 つのスクリーニング戦略について推定した。戦略 1 :「循環器病および腎臓病科」、戦略 2 :戦略 1 プラス「専門的診療科」、戦略 3 :戦略 2 プラス「医療費個人負担の患者」。隔離病床利用率および潜在的 CPE の検出率を推定した。
結果
2014、2015 会計年度における 3 病院の入院 99,105 件のうち、戦略 1、2、3 によりそれぞれ 4,371 例(4.4%)、7,482 例(7.6%)、13,542 例(13.7%)をスクリーニングしたと考えられる。診療科の隔離病床利用率は、戦略、連続検査の回数、先制隔離が適用されたか否かに応じて 3%から 696%までと異なっていた。病院ネットワークでの潜在的 CPE 負荷の予想検出率も 17.1%から 47.5%までと様々であった。
結論
ハイリスク診療科に基づくスクリーニングには潜在的 CPE 負荷のほぼ半分を検出する潜在能力があり、患者レベルのリスク因子に基づくスクリーニングよりも実用的であると考えられる。先制隔離により、隔離の需要は大幅に増加する。CPE スクリーニング戦略では、リスクと資源とのバランスを取る必要がある。

ここに書かれている「カルバペネマーゼ産生腸内細菌科細菌」は、CRE と異なり、確実にカルバペネマーゼ産生が確認されたものである。従って、日本のように CRE を基準としていると、さらに隔離の対象者が増える可能性があり注意が必要である。
