Community-associated meticillin-resistant Staphylococcus aureus: nosocomial transmission in a neonatal unit

市中感染型メチシリン耐性黄色ブドウ球菌（CA-MRSA）は新興病原菌であり、医療関連MRSA（HA-MRSA）獲得の古典的な危険因子（リスクファクター）がない患者で感染を生じているという報告が世界的に増加している。本報告では、英国の医療施設で初めて記録されたCA-MRSA伝播で、新生児室の新生児4例と職員1例が関与した例について記載する。分離菌の詳細な微生物学的特性の調査により、それらは単一クローンであり、multi-locus sequence type（MLST）1型、ブドウ球菌カセット染色体mec（SCCmec）タイプIVa、プロテインA（spa）t127型、agrグループ3、およびエンテロトキシンAとエンテロトキシンH遺伝子をコードしていた。Panton-Valentineロイコシジン（PVL）遺伝子は検出されなかった。CA-MRSA株は、英国で最も優勢なHA-MRSAである流行性MRSA-15の数個のサブタイプと同時に伝播していると考えられた。感染制御策を組み合わせることで集団発生は抑制された。本報告は英国のMRSAの疫学が変化していることを明らかにするとともに、CA-MRSAは市中のみならず医療施設でも発生し得るという事実に医療従事者が注目する必要を強調している。

市中型MRSAの拡大が注目されているが、特に米国ではSCCmec IV、PVL陽性かつパルスフィールド電気泳動でUS300型に分類される流行株が問題となっている。今後の世界的動向が注目されるところである。
