Quality control for handling of accidental blood exposures

標準化されたプロトコール基づいてあらゆる血液曝露事例に対処することを目的に、地域のカウンセリングサービスを設立した。アルゴリズムを用いてリスクの程度を評価した。B型肝炎ウイルス（HBV）、C型肝炎ウイルス（HCV）、およびヒト免疫不全ウイルス（HIV）の伝播リスクのある事例を「高リスク」に分類して、HBVリスクのみの事例は「低リスク」に分類した。リスクの程度に応じて医療介入を行った。1年間にわたりすべての事例を登録し、標準プロトコールの遵守を分析した。2003年に施設が扱った事例は454件であった。このうち36件（7.9％）はリスクなし、329件（72.5％）は低リスク、67件（14.8％）は高リスクと評価された。登録が不完全であったため22件（4.8％）は分類が不可能であった。全体で、HBV伝播リスクのある事例の36％、HCVおよびHIVの伝播リスクのある事例の40％で指定のプロトコールに基づく処置が行われていなかった。プロトコール違反には、対応が不十分なもの（123/396）と過剰なもの（25/396）があった。HIV曝露後の予防については、63件の事例のうち12件で治療が不十分である可能性があった。63件中11件でHCVの追跡調査が不完全であり、396件中5件でHBV免疫グロブリンが投与されず、これには3件の高リスク事例も含まれた。396件の低リスク曝露のうち21件では、報告が遅れるプロトコール違反があった。標準作業手順を用いても、容認可能なレベルの標準的ケアを実施することは困難である。システムの改善のために不備な点の調査と評価が不可欠である。
