Incidence and characterization of Clostridium perfringens isolated from antibiotic-associated diarrhoeal patients: a prospective study in an Indian hospital

ウェルシュ菌（Clostridium perfringens）は抗生物質関連下痢症の全症例の2～15％の原因菌であると報告されており、その診断は、糞便中のエンテロトキシンの検出により可能である。インドの病院でのC. perfringens関連下痢症の罹患率を算出するために、150例の下痢症患者と100例の下痢症でない対照患者を組み込んだ前向き研究を実施した。エンテロトキシンの検出にはC. perfringensの培養、逆受身ラテックス凝集（RPLA）法およびELISAを、エンテロトキシン遺伝子の検出にはPCR法を使用した。Multiplex PCR法により分離菌の型分類を試みた。検査した下痢症患者の糞便サンプル150例のうち、13例の培養が陽性であった。このうちの4例がRPLA法によりC. perfringensエンテロトキシン陽性で、2例がPCR法、また2例がRPLA法およびELISAで陽性であった。下痢症でない対照患者では、27例のC. perfringens培養が陽性であったが、RPLA法またはPCR法でエンテロトキシン陽性のサンプルはなかった。これらの検査法による、C. perfringensによる抗生物質関連下痢症の平均罹患率は2.6％であった。毒素の型分類から、全分離菌がA型に属することが示された。結論として、毒素産生性C. perfringensの罹患率が比較的低かったことから、エンテロトキシン産生C. perfringensはこの集団での抗生物質関連下痢症の主要な原因ではないことが示唆される。
