医療関連または市中獲得型黄色ブドウ球菌(Staphylococcus aureus)菌血症患者 430 例の前向き追跡調査による素因、疾患の進行、および転帰

1999 年から 2002 年に実施された多施設前向き研究により特定した黄色ブドウ球菌(Staphylococcus aureus)菌血症(SAB)エピソード(MRSA を除く)を、感染症専門医が追跡調査した。本研究の目的は、医療関連 SAB および市中獲得型 SAB の素因、進行、および転帰を比較することである。SAB エピソード 430 件中 232 件(54%)が医療関連 SAB であった。医療関連 SAB 患者は市中獲得型 SAB 患者と比較して有意に年齢が高く、罹患期間が長かった。発症から 3 日以内の医療関連 SAB および市中獲得型 SAB の深部感染巣保有率は、深在性膿瘍 26%対 37%(P < 0.05)、肺炎 25%対 31%(有意差なし)、骨髄炎 24%対 36%(P < 0.01)、永久的異物 24%対 9%(P < 0.001)、心内膜炎 11%対 15%(有意差なし)、化膿性関節炎 9%対 13%(有意差なし)であり、感染巣のない患者は 3%対 6%(有意差なし)であった。28 日時点の症例致死率は、医療関連 SAB 14%、市中獲得型 SAB 11%(有意差なし)であった。致死的な転帰の多変量解析による独立したリスク因子は年齢、慢性アルコール依存症、免疫抑制治療、慢性または急性致死性基礎疾患、SAB 発症時の重度の敗血症、黄色ブドウ球菌による肺炎、および心内膜炎であった。前向き研究デザインにより、感染巣の綿密な評価および感染症専門医による各 SAB エピソードの追跡を実施した結果、症例致死率は依然として低く、医療関連 SAB エピソードの 97%には菌血症の発症から 3 日以内に感染巣が認められた。

監訳者コメント
大学病院 5 施設と一般病院 7 施設で行われた、黄色ブドウ球菌による医療関連感染と市中感染の前向き比較研究である。フィンランドでは、黄色ブドウ球菌に占める MRSA の割合が低いため、MRSA 以外の黄色ブドウ球菌感染症の検討を行う価値がある。医療関連感染のほうが市中感染より深在性感染症の頻度が低いという結果は興味深い。
