Cost of intensive care unit-acquired bloodstream infections

集中治療室での血流感染は、重病時の重大な合併症である。本研究の目的は、集中治療室血流感染による滞在期間の延長、死亡率、費用を算出することである。2000年5月1日から2003年4月30日にかけて、カルガリー保健区（Calgary Health Region）のすべての成人用集中治療室を対象として、マッチングによるコホート研究を実施した。集中治療室血流感染患者144例を集中治療室血流感染を合併しなかった患者とマッチングさせた（1：1）。集中治療室血流感染を合併している患者では合併していない患者に比べて、集中治療室滞在期間中央値｛15.5日間［四分位範囲（IQR）8～26日］に対し12日間［IQR 7～18.5日］、P＝0.003｝および病院の治療費中央値［85,137ドル（IQR 45,740～131,412ドル）に対し67,879ドル（IQR 35,043～115,915ドル、P＝0.02）］が有意に増加した。集中治療室滞在期間延長の中央値は2日間であり、集中治療室血流感染に起因する治療費の中央値は1例あたり12,321ドルであった。集中治療室血流感染群の60例（42％）が死亡したのに対し、対照群では37例（26％）であった［P＝0.002、集中治療室血流感染に起因する死亡率16％、95％信頼区間（CI）5.9～26.0％］。集中治療室血流感染患者では、入院中の死亡リスクが増加した（オッズ比2.64、95％CI 1.40～5.29）。生存者同士でマッチングさせたペアでは、集中治療室血流感染発症に起因する集中治療室滞在期間および入院期間の延長の中央値はそれぞれ2日間と、13.5日間であり、集中治療室血流感染による費用は生存者1例あたり25,155ドルであった。集中治療室血流感染を発症した重病患者では罹患率と死亡率が非常に高く、医療費に有意な増加がみられた。これらのデータは、感染予防・制御プログラムに対する支出が必要であり、これらの感染の影響を減少させるためにはさらなる研究を要することを支持するものである。

マッチングによるケースコントロール研究という非常に手間ひまのかかる研究デザインで、十分に検討された論文である。症例数もこの形式の研究デザインでは多い方である。
