カルバペネマーゼ産生菌保有患者の環境の特徴:主要な耐性遺伝子の拡散における空気および表面の役割★★

背景
カルバペネマーゼ産生菌(CPO)による病院関連感染症は、重大な健康上の懸念をもたらしている。医療環境では、CPO の蔓延を抑制しアウトブレイクを予防するための対策が実施されているが、カルバペネマーゼ遺伝子の拡散における空気の役割は依然として明らかでない。本研究では、CPO 保有患者の環境において 3 つのカルバペネマーゼ関連遺伝子(blaKPC、blaOXA-48 および blaNDM)の評価を行った。
方法
カナダのケベック州の 4 つの病院で、CPO 保有患者の病室において前向き観察研究を実施した。CPO 保有患者の病室内で、空気を能動的に収集し、床および非接触表面からウェットスワブおよびウェットスポンジを用いてサンプル採取を行った。その結果を、同一フロアに位置する対照室(すなわち、CPO の非保有患者の病室)の結果と比較した。これらの病室に隣接する廊下からさらなるサンプルを採取して、当該環境内において起こり得る拡散を推定した。カルバペネマーゼ産生遺伝子(blaKPC、blaNDM および blaOXA-48)の有無および存在量を、定量的 PCR を用いて評価した。
結果
カルバペネマーゼをコードする遺伝子は、CPO 保有患者の環境において高頻度に検出され、特に床(検出頻度 97%)、ドアフレーム(52%)、および非接触表面(42%)で多く検出された。これとは逆に、空気サンプルは 1 個のみが検査で blaKPC 陽性であった。これらの遺伝子は、CPO 保有患者の病室に隣接する廊下(92%)、対照室(100%)、ならびにCPOの非保有患者の病室に隣接する廊下(78%)でも検出され、CPO 保有患者の病室からの距離が遠くなるほど遺伝子量は減少した。
結論
これらの結果から、カルバペネム耐性は医療環境内で拡散することができ、また空気は遺伝子拡散に関与する可能性がある。耐性遺伝子の伝播、特に床および空気を介した伝播を制御するために新たな方策を検討すべきである。
監訳者コメント:
空気がカルバペネム耐性菌の伝播に関与している可能性があることを示唆する報告。カルバペネム耐性菌のみらなず、薬剤耐性菌に対する予防策は標準予防策と接触予防策が重要となるが、空気を介するということであれば、多床室でカーテン隔離をしたり、ベッド間隔を空けて、耐性菌非保有患者と一緒に管理するといった苦肉の策は避けた方がいいと思われる。非常に示唆に富む報告である。
