病院の隔離室の定員の前向き評価 ★

個室への隔離の必要性を判定するためにリスク評価が使用されるが、隔離室に限りがあることや運用上の必要性により、リスク評価で妥協をすることがある。本稿では、1,100床の教育病院において、感染制御を目的とするすべての隔離要請について観察した、12カ月間の前向き研究の結果を報告する。さらに、個室使用について4点での隔離実施率調査を行った。病棟ごとの新たなメチシリン耐性黄色ブドウ球菌(MRSA)臨床分離株の発生率についてデータを集め、それらとMRSA症例の隔離不履行率との相関を調べた。患者隔離の要請は845件あり、そのうち185件(22%)は実施されなかった(隔離不履行)。要請の4分の3はMRSAまたはクロストリジウム・ディフィシル(Clostridium difficile)によるものであった。隔離の不履行率はほとんどの微生物や疾患で同一であったが、診療科ごとに大きく異なっていた(0~57%)。隔離不履行の理由は、個室がない、すべての個室/隔離室が使用中(隔離のため、または感染制御以外の理由)、男女混合病棟における個室使用の制限、および患者個別の理由である。使用できる個室のうち、感染制御の理由で使われていたのはごく少数であった(12~19%)。隔離不履行とMRSA発生率には、統計学的に有意な相関があった[Spearmanの順位相関係数(ρ)0.596、P<0.001)]。病床数の30%以上が個室である唯一の病棟では、隔離不履行例が1例あった。結論として、病原体を伝播させる可能性がある患者を隔離するための個室数が不十分なことが多く、感染制御上の要請に妥協がもたらされることがある。隔離室の定員を増やすか、隔離要請が利用可能数を上回る状況に対するエビデンスに基づくリスク評価を行うべきである。隔離不履行の影響について、さらなる情報が必要である。

監訳者コメント:
隔離をする必要があるのに十分な個室のキャパシティがないことは、我々の実地でもよく経験することである。これがもたらす様々な影響として、MRSAの分離率を指標として挙げているが、これは容易に想像できよう。ただし実際にエビデンスベースのリスクアセスメントをどのように変えたら、これが改善したかというステップには到達していない。さらなる発展的研究が望まれる。
