背景

集中治療室(ICU)の血流感染症(BSI)患者における微生物の分布と多剤耐性病原体の存在率の傾向を評価した。

方法

この後向き解析は、韓国国立医療関連感染症サーベイランスシステム(KONIS)からのデータを用いて実施した。BSI 患者における微生物の分布と多剤耐性病原体の年間の傾向を調べた。

結果

黄色ブドウ球菌(Staphylococcus aureusによる BSI の発生率は、2011 年の 33.1/100,000 患者日から 2022 年の 9.7/100,000 患者日に減少した。肺炎桿菌(Klebsiella pneumoniae)による BSI の発生率は、2015 年の 6.4/100,000 患者日から 2022 年の 12.1/100,000 患者日に増加した。メチシリン耐性黄色ブドウ球菌(MRSA)によるBSI の発生率(2011 年に 28.8/100,000 患者日、2022 年に 7.3/100,000 患者日)と MRSA 株の割合(2011 年に 91.8%、2022 年に 76.7%)は、研究期間を通して減少した。イミペネム耐性肺炎桿菌による BSI の発生率は、2011 年の 0.3/100,000 患者日から 2021 年の 6.1/100,000 患者日に有意に増加し、イミペネム耐性肺炎桿菌株の割合は、2011 年の 4.3%から 2022 年の 48.5%に増加した。イミペネム耐性緑膿菌(Pseudomonas aeruginosa)による BSI の発生率は、2011 年から 2021 年にかけて増加傾向を示した。

結論

ICU における MRSA による BSI の発生率は減少したが、ICU におけるイミペネム耐性肺炎桿菌やイミペネム耐性緑膿菌が原因の BSI の発生率は増加傾向を示した。過去数十年間にわたり、BSI においてグラム陽性細菌からグラム陰性細菌への有意な変化があった。

監訳者コメント

韓国からの報告。近年、インバウンドも増える中で、隣国の状況を知っておくことも重要である。MRSA が減ってきており、日本に似たところもあるが、バンコマイシン耐性腸球菌は横ばいであるものの、日本と比べ検出率が高い、イミペネム耐性肺炎桿菌が増え続けている、イミペネム耐性アシネトバクターの検出率が高いなど、日本との相違点もわかり、大変参考になる。
