Compliance with surgical antibiotic prophylaxis － reporting from a statewide surveillance programme in Victoria, Australia

オーストラリアのビクトリア州にある大規模な公立病院で、手術時の予防的抗生物質投与（surgical antibiotic prophylaxis；SAP）のガイドラインに対するコンプライアンスの全州的評価を実施した。本研究は、病院感染のためのサーベイランスシステムの一環として収集されたデータを利用して行った。研究対象は、ビクトリア州の100床以上の公立病院（27病院）で、21カ月間に心臓外科手術、股関節または膝関節形成術、胆嚢切除術、虫垂切除術、結腸手術、子宮摘出術を受けた患者である。オーストラリアのガイドラインでは、本研究に組み入れた10,643例の外科的処置すべてに対してSAPを推奨している。すべての処置のうち87％がSAPを受け、抗生物質の選択は53.3％の処置でガイドラインに準拠し、23.9％の処置で「適切であるがガイドラインに準拠しない」と判断された。18.9％の処置ではSAPが不適切とみなされた。心臓および整形外科手術に対しては、多数の抗生物質レジメンが使用されていた。投与のタイミングは、半数以上の処置で記載されていなかった。投与時期の記録のある処置のうち76.4％がガイドラインに準拠していた。予防的抗生物質の選択は、全般的に心臓および整形外科手術が他の手術よりもガイドラインに準拠していた。しかし、これら診療科におけるSAPでも、死亡率が高い病院では、SAPが不適切な場合があった。病院感染のサーベイランス時に収集されたデータを用いて、SAPのコンプライアンスに関する定期的な報告を行うことは可能である。これにより、コンプライアンスおよび診療記録の改善がもたらされると考えられる。

監訳者コメント：
SAPについてのマニュアル履行率についての調査である。SAPについては、どの薬剤を選ぶかというよりも、どのタイミングで投与をされたかの方が感染率に大きな影響をもたらすことを知っておく必要がある。その上で、投与のタイミングが記載されていないということが、どういう意味をもっているかは、読者が理解しなければならない。
