Control of a cluster of community-associated, methicillin-resistant Staphylococcus aureus in neonatology

新生児病棟における市中感染型MRSA（CA-MRSA）の集団発生を制御するために、新生児および親のスクリーニング、MRSA分離株の分子解析、および症例の長期追跡を含む調査を実施した。2000年夏の2カ月間で、Panton-Valentine型ロイコシジン（PVL）産生CA-MRSA（ST5-MRSA-IV株）を、新生児5例で検出した。初発症例の母親は乳腺炎および創傷感染の徴候を示し、初発症例はCA-MRSAの陽性反応の結果となった。PVL陰性MRSA株（ST228-MRSA-I）による小規模の地域流行クラスターが、同時に発生した。衛生対策の強化、防護的予防策、保菌者の局所的除菌、および新規入院患者のコホーティングにより、集団発生は終息した。集団発生の4カ月後、別の新生児の母親が流行CA-MRSA株による、せつ症を発症した。乳児1例でCA-MRSA保菌が持続し、集団感染の4年後に兄弟の皮膚感染症の原因となった。結論として、流行CA-MRSA株は初発症例の母親が端緒となった。これが新生児の間で拡散し、次いで母親および兄弟に伝播した。本稿は、欧州において制御に成功した、遺伝的に異なるPVL産生CA-MRSA菌株による新生児集団感染に関する最初の報告である。

米国でもEID※でCA-MRSAによるNICUアウトブレイクがすでに報告されており、幸い現時点では、わが国にはPVL産生CA-MRSAはほとんど小児の皮膚軟部組織感染症から分離されていないが、今後、わが国でも小児期のCA-MRSAに関する継続的な監視を行う必要があろう。※Bratu S, Eramo A, Kopec R, et al.
