Lowering standards of clinical waste management: do the hazardous waste regulations conflict with the CDC’s universal/standard precautions?

医療廃棄物は費用のかかる、厄介な代物である。医療行為から生じる廃棄物における最大の危険は、これらの廃棄物に存在する微生物による感染のリスクである。傷のある皮膚への汚染や眼に入った飛沫が感染を伝播することもあるが、一般的に感染は穿通損傷、いわゆる「鋭利物」または「針刺し」による切創を介して生じる。呼吸器感染、軟部組織感染、および腸管感染もないわけではないが、血液媒介ウイルス［B型肝炎ウイルス（HBV）、C型肝炎ウイルス（HCV）、ヒト免疫不全ウイルス（HIV）］は、最も深刻な脅威である。廃棄物の分類や規制の調整を図る欧州有害廃棄物指令（European Hazardous Waste Directive）は、感染リスクが低いと思われる施設での医療廃棄物の規制緩和を認めた。英国の規制ガイダンスは、廃棄物分類のリスクアセスメントに対する要請に基づいて強化されているが、一部の医療廃棄物については危険性はないものと指定する方向にあり、これは米国疾病対策センター（CDC）のユニバーサルプリコーション（普遍的予防策）、すなわち医療現場におけるHIV、HBV、およびその他の血液媒介病原体の伝播を予防し、医療従事者などの血液媒介ウイルス感染を予防するための対策、およびこの基本方針を医療施設関連感染および院内病原体の環境拡散の予防にまで拡張した、より包括的なスタンダードプリコーション（標準予防策）を完全に無視するものである。欧州有害廃棄物指令に基づく英国の法規は、強力なコスト推進要因を創出することによって一部の医療廃棄物の規制緩和を助長するものであり、CDCのスタンダード・ユニバーサルプリコーションとは矛盾している。

医療廃棄物は、もはや一国の問題ではなく、国際的な問題となってきている。本論文は、特に地続きの欧州ではEUによって廃棄物まで含めた流通が広域化し、様々な政治的圧力の中から将来の方向性を模索している現状を紹介しているので、一度熟読して自分なりに考察されることをお勧めします。
