病院感染症の有病率調査において外因性危険因子(リスクファクター)の存在を調べる際には、様々な困難に直面するために複数の方法論によるアプローチを利用しなければならない。本研究では、病院感染症に関する外因性の危険因子への曝露に関するデータを収集する方法として、後向きアプローチの有効性を評価した。1990年にデータ収集されたEPINE(Study of the Prevalence of Nosocomial Infections in Spain;スペイン病院感染症発生動向調査)データベースからシミュレーションモデルを作成した。評価したすべてのアプローチにおいて、危険因子と感染症の関連性の程度は異なっていた。調査実施日における危険因子への曝露率、調査実施前の曝露率、および全曝露率は、曝露の有無および暴露期間と有意に関連していた。曝露期間が4日未満の場合、調査前の週に曝露があった患者の有病率は、調査の日に曝露があった患者よりも高かった。危険因子への曝露を後向きに調査したところ、実際に曝露があった日より以前の日数を何日間、研究に組み入れるべきかを決定すべき根拠を見いだすことはできなかった。病院感染症が原因でデバイスを抜去する頻度は感染部位によって異なるが、そのような頻度は本研究ではあまり高くなかった。過去の曝露を調べずに、調査日のみに危険因子の存在に関するデータを記録するという制限下においても、調査当日の点有病率調査における外因性危険因子の負荷を適切に推計することができる。

監訳者コメント:
この論文はかなり難解でありサマリーの邦訳のみでは理解しづらいが、結論として、血流感染症については中心静脈カテーテル、末梢挿入型中心静脈カテーテル、末梢静脈カテーテルによる非経口的栄養管理、尿路感染症については開放式または閉鎖式の尿道留置カテーテル、肺炎については気管切開と人工呼吸器管理がそれぞれ外因性危険因子として調査されており、これらの器具使用の状況について、調査当日のみのポイント調査でも、病院感染症の発生率を評価するための分母として用いてよいことを主張している。本来的には医療関連感染症サーベイランスは前向きに実施されるべきであるが、1日調査のみでも、ある程度の発生率は把握できるであろうとする主張に正当性があることが示された。
