小児血液腫瘍患者における埋め込み式ポートおよびヒックマン・カテーテルによる感染性合併症 ★

本研究の目的は、小児におけるヒックマン・カテーテルおよび埋め込み式ポートに関連する感染性および非感染性合併症を比較検討することである。研究は、イスラエルSchneider子ども医療センターの血液腫瘍科で3年間にわたって実施した。中心静脈カテーテルを必要とする全患者を本研究の対象とした。カテーテル関連血流感染症の各エピソードについて、人口統計学的、臨床的、および微生物学的データを記録した。試験期間に、419の皮下トンネル型中心静脈カテーテル(埋め込み式ポート246およびヒックマン・カテーテル173)を281例の患者に挿入した。ヒックマン・カテーテルは埋め込み式ポートと比較して血流感染症発生率が有意に高く(1,000カテーテル・日あたり4.656エピソード対1.451エピソード)、初回感染症発生までの期間が短く(52.31日対108.82日、P<0.001)、カテーテル留置期間が短く(140.75日対277.28日、P<0.001)、機械的合併症による抜去率が高かった(P<0.005)。グラム陽性細菌感染症は埋め込み式ポート群のほうが多いのに対し(63.6%対41.6%)、グラム陰性桿菌、複数菌感染症、および抗酸菌感染症はヒックマン・カテーテル群のほうが多かった(それぞれ31.4%対50.9%、17%対36%、および0%対4.4%;すべてP<0.05)。造血幹細胞移植は感染症発生[オッズ比(OR)1.68、P=0.005]および合併症によるカテーテル抜去(OR 2.0、P<0.001)の独立した危険因子(リスクファクター)であることが示された。埋め込み式ポートは、ほとんどの腫瘍患児および幹細胞移植患児にとって好適な器材であると考えられる。

海外では長期的な中心静脈カテーテル管理が必要と考えられる場合はカフ付きの皮下トンネル型が使用されることが多く、ヒックマン・カテーテルはその代表的なデバイスである。この論文では皮下トンネル型中心静脈カテーテルとしての埋め込み式ポートとヒックマン・カテーテルの両者について感染率を比較し、埋め込み式ポートの優位性を報告している。末梢挿入型中心静脈カテーテル(peripherally-inserted central venous catheter;PICC)など、わが国の中心静脈カテーテル管理に取り入れるべき器材は少なくない。
