院内感染菌血症患者505例を対象に1997年と2002年に実施した全病院規模の前向き研究 ★

院内感染菌血症の発生率、感染源、および転帰に関する調査を1997年と2002年に実施した。血液培養の結果を連日検討した。入院後48時間以降に採取した血液の培養結果が陽性であった患者すべてを研究対象とし、医療記録を検討した。感染源は、事前に定義した臨床的基準と微生物学的基準の両方または一方により判定した。患者は退院または死亡まで追跡した。コンタミネーションを除外した患者由来の菌血症の診断は1997年に851例、2002年に857例であった。そのうち228例(27%)と277例(32%)が院内感染であった(P<0.05)。全体の発生率は1,000入院あたり7.5から7.0に低下した(P<0.001)。菌血症の原因は、1997年と2002年でそれぞれ、血管内留置カテーテル36%と27%(P<0.05)、尿路感染8%と15%(P<0.05)、気道感染5%と13%(P<0.01)、手術部位感染14%と4%(P<0.001)であった。患者の3分の1で菌血症の感染源を特定できなかった。患者の生存退院例はそれぞれ52%と54%のみであった(有意差なし)。1997年に最も多かった分離菌は黄色ブドウ球菌(26%)で、次いでコアグラーゼ陰性ブドウ球菌(13%)、Klebsiella pneumoniae(11%)であった。2002年には黄色ブドウ球菌の頻度は11%に低下し(P<0.001)、Acinetobacter属が単独では最多の分離菌となった(6%から17%に増加)(P<0.001)。Acinetobacter属による菌血症に関連する病院内での死亡率(57%)は、他の病原菌による死亡率(31~43%)と比べて有意に高かった(P<0.05)。
結論として、院内感染菌血症の前向きサーベイランスによって有益な情報が得られ、有効な介入を追跡することが容易になった。
