教育病院における抗生物質使用と環境微生物の耐性の関連

標準化した4カ月の環境スクリーニングプログラムを、集中治療室(ICU)、急性期脳卒中治療室、および内科日帰り治療室で実施した。これらの病室由来の環境微生物を調査し、抗菌薬の使用に関連した細菌の耐性を比較することを目的とした。市販のディップスライドを用いて、手指の接触のある場所や、その他の場所のスクリーニングを行い、職員には指先の培養を提出するように依頼した。患者の血液試料は研究期間を通して保存した。抗菌薬感受性試験などにより、微生物を量的・質的に評価した。抗生物質使用量に関する1,000患者・日あたりの1日規定量の前年分のデータを、各治療室から入手した。ブドウ球菌属276例とグラム陰性桿菌67例を分離した。ブドウ球菌属(P<0.0001)と大腸菌群(P=0.04)の抗生物質耐性は治療室の種類と有意な関連があり、少数ではあったがその他のグラム陰性菌でもその傾向が認められた(P=0.06)。ICUでの抗生物質使用量は、急性期脳卒中治療室および内科日帰り治療室の6倍であった。特定の抗生物質群の使用量と、それらに対するブドウ球菌属およびグラム陰性桿菌の耐性の間には関連が認められた。それぞれの病室由来の環境細菌における唯一の有意な相違は抗菌薬耐性であったが、これは処方による圧力の反映であると考えられた。病室の目視検査からは、病院環境における多剤耐性微生物の有無や潜在的な感染リスクに関する的確な指標は得られないと考えられる。これらの知見は、部署レベルでの抗生物質の使用方針・感染制御・清掃計画に意義を有するものである。
